Summoner Spells That Fit

The reasoning for each is on the summoner spells page. You may only bring one per match, and the best choice shifts with the enemy draft.

Saint holds the highest Magical Attack in red at +5.3; Nightmare trades some of that for early penetration.

Counters & Synergies

Countered-by and synergy picks are this site's own reading, based on each hero's kit. What the official app publishes is how often heroes are picked together (see "Frequently Paired With" below); it does not publish any rating of how well they match up.

Frequently Paired With

3-hero team

The figures are the "match rate" published by the official in-game HoK Camp: how often these heroes ended up on the same team (fetched 2026-08-14). It is not a win rate, so a higher number does not mean a stronger pairing.

Strategy Guide

Playstyle

A dual-ledger mage who stacks poison marks on enemies and healing marks on allies, then detonates both at once. Every decision moves two scoreboards simultaneously.

Best for: Players who see offense and defense as one board — and love flipping a fight with a single well-timed detonation.

Recommended Combos
Wait for Five Stacks
Skill 1 (poison pool) → Skill 2 (damage + team heal) → Basic Attack → Basic Attack → Skill 3 (detonate every mark)

Each basic attack or skill hit adds one poison mark, capped at 5 stacks. Every stack raises the detonation's damage and healing by 50%, so two stacks and five stacks are entirely different weapons. Don't rush it — fire when your team's health starts to drop.

Crack a Pool and Walk
Skill 1 (at your own feet) → Skill 2 → Skill 3 (extra self-heal)

With no mobility skill, the answer to a dive is to make them stand in poison rather than to run. The pool lasts 4s and slows anything inside it by 30% for 2s. Skill 3 detonates the marks and restores an extra 400 (Lvl 1) Health to Dr Bian himself. That is the walk back to your team.

These combos are not official data. They are this site's own write-up, built only from the skill data transcribed from the in-game display; nothing that the skill text does not state is included.

Strengths
  • Poisons enemies and heals allies at the same time — long fights snowball his way
  • Five-stack Ultimate delivers big AoE damage and a team-wide heal in one cast
  • Three stocked vials make him excellent at zoning objective pits
Weaknesses
  • No mobility skill — an assassin who reaches him rarely lets him leave
  • Needs time to stack marks, so instant-burst shootouts are not his game
  • Anti-heal items erase half of what he brings
Early Game Strategy

Dr Bian's early game is an exercise in patience. With no burst, mid lane trades are losing propositions until your first core item finishes, so clear waves with Skill 1's poison pools and manage your HP. Vials restock every 12.5 seconds up to a cap of three — always bank one for self-defense. Whenever the enemy jungler is off the map, dropping a pool at your feet before backing off cuts gank success rates dramatically. Basic attacks apply a poison stack through the Passive too, so keep tapping targets from safe range.

Mid Game Strategy

Around 8 minutes, with one or two items done, you can drag fights into your arena: long skirmishes. In Tyrant standoffs, throw Skill 1 at their feet from behind your frontline, cycle Skill 2 for chip damage and ally healing, and once poison marks build up, detonate them with your Ultimate. Each stack adds 50% to the blast's damage and healing, so holding for 5 stacks instead of panicking at 2 changes what the fight looks like. Past the 10-second mark of any brawl, the poison-versus-healing math starts sinking the enemy team.

Late Game Strategy

The later the game, the more Dr Bian wants to dodge coin-flip bursts and drag everyone into attrition. Line up three poison pools in front of Shadow Tyrant or Overlord and the enemy either fights while melting or gives up the spawn. In teamfights, stand near the center of your team and use the Ultimate's detonation for AoE damage and a team-wide heal in the same instant. Once enemies start buying anti-heal items, your edge shrinks — compensate with timing, detonating earlier, before allies drop below half HP.

Teamfight Strategy

Skip the flashy opener — you're the fight's accountant, keeping the books on marks. Open with Skill 1 into their clump, throw Skill 2 at whoever is easiest to hit (usually the frontline) while healing allies around you, and once both teams tangle, detonate everything with the Ultimate. Blowing 5-stack poison across multiple enemies at once is the comeback button: AoE damage and a team recovery in the same moment. When an assassin jumps you, crack a pool at your own feet and walk them across the 30% slow toward your team — that alone changes your survival rate.
